Position Description:
Continuum Hospice Care/Jacob Perlow Hospice is an open access hospice.
We are seeking Hospice Homecare Case Manager Nurses who are Responsible for the provision of palliative nursing care for the terminally ill and coordination of the interdisciplinary patient care plan in an environment where the emphasis is placed on both patients’ and their families’ physical, psychosocial, emotional and spiritual needs. Hospice Nurses implement skilled and supportive nursing services as outlined in the care plan, especially in those areas involving the use of medications for the management of palliative pain and distressing symptoms. In conjunction with other members of the interdisciplinary team, nurses provides ongoing education to patients, caregivers and other family members regarding treatment choices and goals; use of equipment and services; disease symptom progression; truth-telling; forms of communication; and the expression of grief and loss
Requirements:
Must possess and maintain a current license as an RN in the state of New York.

Hospice Homecare Nurses do not work on a unit or in a single facility.
Hospice Homecare Nurses visit patients at their homes in the community, some of which are skilled nursing facilities.
Full-Time, non-union. General hours vary - some flexibility for weekends and holidays required. Majority of work week will be Mon - Fri, 5 days/per wk.
