POSITION SUMMARY:
In accordance with Ministry Home Care policies and procedures collaborates with case manager to provide discipline specific care. Cooperates in and completes the full range of care and documentation responsibilities associated with the discipline. Maintains or exceeds professional practice standards and the professional code of conduct.
ESSENTIAL JOB FUNCTIONS or RESPONSIBILITIES:
• Understands and assures regulatory compliance issues.
• Demonstrates efficient and effective use of available resources.
• Participates in Quality Improvement measures to improve outcomes.
• Provides education to patients, families and agency personnel.
• Participates in discharge planning activities.
AGE OF PATIENT CARED FOR: Birth through Geriatric
Qualifications
• Education: Bachelor’s degree in physical therapy from an accredited university
• Experience: Minimum 1 year clinical experience
• Licensure: Licensed as a physical therapist by the State of Wisconsin
• Certification: CPR certification
• Skills: Communications/Interpersonal Skills
• Other: Valid driver’s license, use of car and proof of auto insurance required
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:
This position has physical requirements that require a full range of body motion including, but not limited to, physically assisting patients, requiring stretching, bending, lifting, sitting, standing, walking, pushing and pulling, manual and finger dexterity and hand-eye coordination. Vision and hearing must be correctable to the normal range. Work may be performed in a medical office environment with potential exposure to communicable diseases and other conditions common to the clinic environment.
