Description:
Accurately completes the MDS and the care plan process to assist staff in providing a plan to give quality care to the residents and to ensure that each resident is maintained at their highest practicum level
Duties:
Completion of the MDS
• Resident care assessments
• Supportive Documentation
• Documentation of findings
• Transmission of the MDS
Completion of Care Plans
• Compose care plans after the MDS completion
• Compose care plans from the resident assessments
• Compose care plans from resident status
• Schedule and attend care plan meetings
• Update care plans as needed
Ensure proper documentation
• Monitor medical records for supportive documentation
• Compose policies and ensure that proper forms are being used for documentation
• Monitor for completion of weekly and monthly summaries
• Monitor for completion of monthly care plan charting
• Monitor Nursing Assistants documentation for completeness
• Staff education
Nursing Rehabilitation
• Assess and evaluate the need for nursing rehabilitation
• Compose a plan of care and inservice staff to meet goals
• Follow up on plan to ensure that it is being completed by all staff
• Staff education
• Resident education
• Report status to the Director of Nursing weekly
• Report status to the Quality Assurance Team quarterly
Quality care of residents
• Care planning
• Assist with resident care as needed
• Assist staff nurses as needed
• Work as staff nurse as needed
• Report concerns to appropriate staff member
• Staff education
• Call status as needed
Infection Control
• Monitor all infections in the Pavilion and Courtyard area
• Document infections, treatments and outcomes
• Monitor for clusters in infections
• Report information to the Director of Nursing monthly and as needed
• Report information to the Quality Assurance Team quarterly
Quality Assurance
• Monitor quality indicators
• Compose plan to decrease flags in the quality indicators by education of staff/residents, care planning and assessments
• Report findings to Director of Nursing monthly and as needed
• Report findings to the Quality Assurance Team quarterly
• Participates in survey inspections
Medicare Assignment
• Evaluate all new residents for need of Medicare coverage
• Inform Healthcare team if Resident meets criteria for Medicare coverage
• Monitor days of Medicare
• Meet with therapies daily
• Meet with the Medicare team
Qualifications:
Our position requires a Registered Nurse who has long term care experience with demonstrated supervisory, customer service and public relations skills. Two years or more of MDS experience required as well as familiarity with admission process in long term care. The successful candidate will possess strong assessment and customer service skills and will be skilled in delegating and prioritizing responsibilities
