JOB DESCRIPTION

PURPOSE: Assumes the management of the patient care of a designated high-risk patient population during an entire episode of illness; provides ongoing assessment and monitoring of the patient's clinical, functional, and psychosocial status; and provides the communication link for coordination of care among the multidisciplinary health care team within and outside the program. The case manager is responsible for assisting in the development, implementation, and evaluation of a program of health care services for a defined patient population. The nurse coordinated model, which is cost effective, accountable and provides quality outcomes. Patient care includes care to patients in the following age groups: neonatal (birth to 30 days), pediatric (30 days through 12 years), adolescent (13 through 18 years), adult (19 through 64 years), and geriatric (65 years and above).

PRIMARY FUNCTIONS - JOB DUTIES: The percentage of time spent performing primary functions is 85 %. Qualified individuals must have the ability--with or without reasonable accommodation--to perform the following duties:

1. Manages the care of a high-risk patient population, evaluates patient care outcomes, plans corrective action for identified variances, and utilizes a system to collect and aggregate pertinent data.

2. Collaborates with nursing staff, physicians, and other members of the health care team to achieve patient outcomes within allotted time period utilizing unit, hospital, and outpatient resources in an effective and efficient manner.

3. Serves as a role model and provides clinical consultation and education for the members of the multidisciplinary team related to nursing care, expected patient outcomes, and resource availability.
