DESCRIPTION:
The Admission Nurse performs pre-admission evaluations for appropriateness for hospice services. Initiates the admission process by educating and informing patient/family of hospice and its services. Obtains informed consents. Provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life.
OUALIFICATIONS:
Registered Nurse currently licensed in the state(s); previous experience in hospice, home health, geriatrics or general medical surgical nursing; demonstrated commitment to hospice values; willingness to work a flex schedule of 40 hours a week and serve patients/families in the geographic service area; detail oriented with good time management skills; above average computer documentation skills; management/leadership skills.
