I was excited to read about Chronic Disease Case Manager job opening at CGI Medical.
I have bachelor degree in health with Current California Registered Nurse license, Certified Diabetes Educator, Registered Dietician and Certified Asthma Educator. I have 5 years experience in a health care environment; managing the care of chronic diseases (diabetes, asthma, etc.) and one year experience in case management with the tasks, duties and responsibilities focusing on:
• Works in partnership with disease management and case management staff to develop, implement and maintain health and disease management initiatives and programs;
• Develops collaborative relationships with other health care partners to facilitate and support quality chronic disease education and teaching services;
• Develops strategies to address compliance issues for members at risk;
• Provides member support and outreach;
• Provides counseling and referrals over the telephone to select health plan members with high-risk conditions such as, but not limited to diabetes and asthma, as well as those who demonstrate high ED use;
• Collaborates with Quality Improvement Manager to track and prompt for services due (HgA1c, annual eye exam, etc.);
• Collaborates with senior management regarding decisions to meet business needs and initiatives related to health and disease management projects;
• Scans the medical literature routinely for updates on clinical practice guidelines, new developments in the management of chronic diseases, benchmarking of best practices in others managed care environments, and communicates and disseminates information to internal and external professional clinical audiences as well as translating technical clinical findings into understandable information for members;
• Evaluates the chronic disease education program on a continuing basis;
• Monitors and evaluates short and long-term member responses to interventions; tracks and maintains necessary follow-up for members.;
• Participates in quality improvement activities involving chronic disease and health management programs;
• Makes internal and external social services assistance referrals as necessary;
• Ensures the development of culturally sensitive services to chronic disease members;
• Maintains records of member and provider contacts, identifying problems, interventions, goals and outcomes;
• Participates in plan Case Management meetings dealing with medically complex members;
• Participates in Case Management activities with other agencies as requested and as appropriate;
• Monitors and reports on plan population based chronic disease management outcomes;
• Participates in annual HEDIS studies with emphasis on chronic disease measures;
In addition to my extensive experience, I Speak Bilingual (English/Spanish); Bi-literate (English/Spanish); Bicultural (English/Spanish); Strong clinical background; Case management experience; Ability to communicate effectively both orally and in writing; Ability to use MS Word; Excel and PowerPoint; Knowledge of Medi-Cal and related Title 22 regulations; Ability to evaluate medical records and other health care data; Ability to exercise good judgment and tact in relating to health care providers & beneficiaries; Ability to establish and maintain effective and cooperative working relationships with Alliance staff and others contacted in the course of the work; Ability to accurately complete tasks within established timeframes; Ability to effectively prioritize multiple tasks and deadlines; Ability to maintain confidentiality; Valid California Driver License, transportation and automobile liability insurance in limits acceptable to the Alliance.. My broad background makes me an excellent candidate for Chronic Disease Case Manager position.
