I was excited to read about Pediatric Asthma Program Coordinator job opening at Chelsea Health Center.

I have BS degree and speak Bilingual in Spanish and English. I have three years of experience working in community settings, working as a patient advocate/outreach worker and accessing community resources with the tasks, duties and responsibilities focusing on:

• Conduct an initial health interview with patients, relatives and case-related professionals to obtain history relevant to the management of those with asthma. Identify and address barriers to care, reinforcing physicians¿ guidelines regarding asthma self-care.
• Conduct follow-up phone calls with patients to ensure asthma management.
• Communicate with clinicians caring for the patients in the program. Document work in electronic medical record.
• Conduct outreach to patients who are not receiving key preventive health initiatives ¿ i.e. flu shots and routine visits with primary care provider. Schedule appointments for follow up and ensure that patients keep appointments.
• Obtain care plans from primary care providers and reinforce them with patients and their families.
• Provide culturally sensitive services to patients from many different cultures. While the programs primary target is Latino patients, work should be done to reach out, connect with and help to navigate patients from many different cultures.
• Contact and track all patients over time. Maintain accurate records of patients participation in program in program data base.
• Make home visits. Assess the factors that hinder a patient proper asthma management. Work with primary care providers and school nurses towards solutions of these problems.
• Attend community forums to provide health education on asthma prevention and management.
• Outreach to the City of Chelsea Public Health Department, the Public School Department, school nurses and some community agencies as well as other primary care providers in Chelsea to establish better collaboration and city-wide efforts to address asthma.
• Conduct educational/support groups for elementary school children living with asthma.
• Regularly review Urgent Care and other logs to detect cases that have required emergency care. Contact these families to help arrange follow-up care, and to review correctable factors which may have contributed to worsening illness.
• Act as a liaison and patient advocate for patients and their families with other agencies and health care institutions.
• Secure and coordinate hospital and community resources necessary for the control of asthma (e.g. health insurance, assistance with medications, equipment and allergy control materials, improvements in housing or work conditions, etc.).
• Work towards appropriate communication of the program giving presentations at meetings and conferences and writing summaries and formal reports.
• Collect and revise information on community health and welfare resources.
• Collect and help analyze community-wide statistics concerning asthma.
• Facilitate environmental inspections and remediation plans for home and school.
• Facilitate quality improvement efforts in clinical units Pediatrics and Urgent Care.

In addition to my extensive experience, I have Ability to be creative and work towards further program development, Ability to work both independently and as a team member in multicultural settings, Ability to communicate effectively with all staff and clients, Superior oral and written communication skills, Detail-orientated with the ability to multi-task, Data collection skills, Proficient in all Microsoft Applications, including MS Office and PowerPoint, Knowledge of electronic medical records a plus, Skills in the use of computer spreadsheets and database applications, Strong time management and planning skills. My broad background makes me an excellent candidate for Pediatric Asthma Program Coordinator position.
